Payer Housing Partnership Addresses Social Determinants of Health

Health Partners Plans and two community programs in Philadelphia have announced a partnership to address housing-related social determinants of health by connecting homeless individuals to healthcare and other services, HealthPayer Intelligence reports. The partnership comes on the heels of a survey by Change Healthcare that found 80% of payers incorporate interventions around the social determinants of health into their population health management initiatives.
